Patients due an annual review with a nurse to help with the management of a long term condition can now book their appointment via the NHS App.
Annual reviews are offered to patients in relation to long term conditions such as high blood pressure, heart disease, heart failure, stroke and diabetes. Patients are contacted by the Practice each year when they are due their annual review, this is normally timed around their birth month.
These appointments have been added to the NHS App in addition to other types of appointments including:
- Non-urgent GP appointments.
- Flu vaccinations.
- Smear tests.
- Asthma and COPD reviews.
When booking an appointment via the NHS App
Select the type of appointment you would like, always choose a Practice site location (this can be any of our four sites and does not need to be your usual Practice site) and follow the steps to book your appointment.
Cancelling an appointment
Once you have booked your appointment, if you find you no longer need it, you can cancel it via the NHS App.
Patients will also receive a reminder text message from the Practice before the appointment, which also enables the appointment to be cancelled if it is no longer needed.
Getting started with the NHS App
The NHS App allows you to access a range of NHS services including ordering repeat prescriptions.
If you haven’t yet downloaded the NHS App, it is available on iOS and Android on your smart phone or tablet. You can also access the same services in a web browser by logging in through the NHS website.
You can complete the whole registration process without having to contact the Practice, as the App has facial recognition software that uses your passport or driving licence as a way of identifying you.
